Fees are based on usual and customary fees at the time of service. If you pay for your treatment with CareCredit, the discount will be 15% rather than 20%.

• Smile Design Dental Care Club is not dental insurance and can only be used at Smile Design Dental.

• Membership is non-transferable.

• The 12-month membership fee is due in full at the time of joining before any discounts can be applied.
• Membership is effective on the date of purchase.
• Treatment requires a 50% down payment at scheduling. The remaining balance is due at the time of service.
• It is the member’s responsibility to schedule all visits within the 12-month period to maximize benefits.
• Unused benefits expire at the end of the term. Benefits do not roll over and are not refundable.
• Your renewal month is the month you originally joined. The patient is responsible for renewing.
